Transitional Support (Phase 2 Program)

The Phase 2 Program also serves families who initiated services in our Emergency Shelter Program. These families have accessed their own housing and are now struggling to maintain independence. These families continue to receive structured case management, mental health counseling, financial counseling and parenting and life skills education and assistance with basic needs and emergency transportation. Emphasis is placed on financial planning for the future; clients are included in educational seminars offered through the agency. The services continue for 18 months after the family reaches their own housing. The specific goal of Phase 2 is to maintain permanent housing and prevent relapse(s) into homelessness. We initiated this service component in 2003 and have been tracking the status of the families in this program since its inception. At this time, our families are maintaining their independence when national statistics indicate that 90% of families who have been in emergency shelters will relapse into homelessness within the first year.

In 2013, 14 families were served in our Phase 2 Transitional Support Program, 37 individuals 21 were children and parents were 13 single mothers. The families received 747 hours of professional case management and support, 11 parents were employed, 2 parents were disabled and all were from Somerset County.
